What VisitAhead provides
VisitAhead provides printable and fillable PDF tools to help you organize information, prepare questions and record plans discussed with your healthcare provider. Choose only the pages that help you. No subscription, clinical service or ongoing monitoring is included with a PDF download.
Medical disclaimer
VisitAhead does not diagnose, treat, recommend medications or provide medical advice. Using a worksheet or contacting us does not create a healthcare-provider relationship. The tools do not replace professional advice, medical records or your provider’s instructions. Confirm anything unclear with your healthcare provider. Do not delay seeking care because of content in a worksheet or on this website. For an emergency, contact local emergency services.
Your permitted use
You may download, save, fill in and print the free Starter Pack for your personal use. You may share the link to its official download page. A paid toolkit purchase, when available, gives one person a non-exclusive personal-use licence. You may reuse and print its pages for your own appointments, and a caregiver may help you complete them.
You may share your completed pages with your healthcare providers or caregivers. You may not resell, redistribute, publicly upload or rebrand blank paid templates, or use them commercially without written permission. Copyright in the templates and branding remains with VisitAhead or its licensors. Your personal information and worksheet answers remain yours.
Files, devices and accessibility
Downloads are digital PDFs; no physical product is shipped. A compatible PDF reader is needed to fill and save fields. Features can vary by device and app. Before an appointment, test that your entries save correctly or print and write by hand. You are responsible for keeping and backing up your completed files. Contact us if a format or accessibility issue prevents you from using a pack.
Paid downloads, delivery and refunds
Paid checkout is currently closed. When sales open, the product page and checkout will identify the file, price in Canadian dollars, applicable taxes, payment options and delivery method before you order. Downloads are digital PDFs, with no physical shipment. A one-time purchase does not include a subscription or a promise of all future versions.
No change-of-mind refunds after download. Because a downloaded PDF cannot be returned, we do not provide refunds simply because you change your mind after downloading it. Review the product description, preview pages and free Starter Pack before purchasing.
If you have not downloaded the file, contact us promptly to request cancellation before downloading. We will check the delivery or download status before deciding whether a discretionary cancellation is available.
If you receive the wrong file, a damaged file or a file you cannot access, email sales@visitahead.com with your order reference and a short description. We will first help you access the correct working file or provide a replacement. If the problem cannot reasonably be resolved, we will provide a refund or another remedy required by law. Duplicate or incorrect charges will be investigated and corrected.
Do not send payment-card details, medical information or completed worksheets with a refund request. Any refund will be returned through the original payment method; processing time depends on the payment provider and your bank. Nothing in this policy limits cancellation rights, guarantees or remedies you have under applicable consumer law.
Privacy and communications
Read our Privacy Policy for information about website visits, support messages and orders. Do not submit completed worksheets or sensitive medical information through our contact form or email. We do not monitor messages for urgent health concerns.
Responsible use and external links
Do not misuse the website, interfere with its security or infringe another person’s rights. External links are provided for convenience; their operators control their content and practices. We cannot guarantee uninterrupted website access or particular health or appointment outcomes.
Applicable law and changes
These terms are subject to applicable law and mandatory consumer protections, including those that apply where you live. We may revise these terms for future use and update the date above. Changes do not remove statutory rights or retroactively change an existing purchase.
